Medial malleolar osteotomy and talar osteochondral fragment fixation using Inion CompressOn™ screw

This surgical technique video shows you the animated version of a medial malleolar osteotomy and talar osteochondral fragment fixation using Inion CompressOn™ screws. Inion CompressOn™ is a bioabsorbable headless cannulated screw that can be used in a wide range of indications in the areas of upper and lower extremities.

Surgical Technique – Step by Step

Medial Malleolar Osteotomy and Talar Osteochondral Fragment Fixation

  1. Insert the K-wires before performing the medial malleolar osteotomy.
  2. Perform the osteotomy with an oscillating saw.
  3. Prepare the bone surface under the detached osteochondral fragment with a rasp and K-wire.
  4. Insert the K-wires and measure the depth.
  5. Advance the drill tap to the bone through the drill sleeve. Read the depth markings on the drill tap. Ensure the bone hole is long enough and select the appropriate screw.
  6. Remove the drill tap from the bone carefully using reverse rotation.
  7. Rinse the screw hole.
  8. Attach the Inion CompressOn™screw (e.g. OSH-2716) to the screwdriver shaft. Ensure that the screwdriver is fully inserted in the screw head socket. There should be no “metal” surface visible between the screw head and depth marking when the screwdriver is properly inserted in the screw head socket.
  9. Insert the first Inion CompressOn™ screw fully into the bone over the K-wire.
  10. Advance the drill tap to the bone through the drill sleeve. Read the depth markings on the drill tap. Ensure the bone hole is long enough and select the appropriate screw.
  11. Remove the drill tap from the bone carefully using reverse rotation.
  12. Rinse the screw hole.
  13. Insert the second Inion CompressOn™ screw fully into the bone.
  14. Reattach the osteotomy fragment.
  15. Drive the K-wires to the desired depth in the bone. Measure the depth.
  16. Advance the drill tap to the bone through the drill sleeve. Read the depth markings on the drill tap. Ensure the bone hole is long enough and select the appropriate screw.
  17. Rinse the screw hole.
  18. Attach the Inion CompressOn™ screw (e.g. OSH-4545) to the screwdriver shaft. Ensure that the screwdriver is fully inserted in the screw head socket. There should be no “metal” surface visible between the screw head and depth marking when the screwdriver is properly inserted in the screw head socket.
  19. Insert the Inion CompressOn™ screw fully into the bone over the K-wire.
  20. Stabilize the osteotomy with two Inion CompressOn™ screws.
